Included services:
- Covers one adult (male or female)
- Initial consultation & intake provided by provider
- Hormone testing (blood, saliva, or urine spot testing) in home
- Testosterone injections or pellet therapy (pellet therapy would need to be pre-paid and requires a three-month basic contract)
- RX including: Anastrozole, HCG injections, Progesterone / Estrogen (female patients) if needed
- Recommended vitamins and supplements dispensed in 30-day supply (customizable)
- Four (4)Â IV Vitamin bags of patient choice (1 a week)
- Oxygen therapy dispensed with IV infusion and TP draws
- De-stress neck pillow with essential oils
- Lab draws or therapeutic phlebotomy as indicated by treatment plan (CBC monthly), HemoCue with onsite results
- Chronic disease management such as monitoring of HTN, DM, high cholesterol, arthritis, skin conditions, and others at the medical director’s discretion not fully listed. If complex or abnormal, we may refer to a specialist for treatment and management. Certain conditions apply with the approval at the discretion of the medical director
- Vital & weight monitoring
- One sick visit a month – PRN separate from HRT injection visits
- Four visits a month with providers – one additional sick visit, PRN may be performed by telemedicine visit
